Narrative

At 11:35 AM employee was shoveling at the C-fine chutes at the Metcalf secondary crusher. As he reached the top of the stairs, he slid backwards into an 8"" opening next to a 36"" section of duct work. As a result, his lower extremities were caught between duct work and metal flooring lodging him in place until he could be physically removed.
