Employee was loosening a fitting on a 2"" pipe and noticed a pain in his lower back. Employee contacted medical provider by phone and was given home care instructions. The employee called again on 7/5/08 and still had tightness in his lower back. He was seen by a physician on 7/7/08 and prescribed medication for the muscle strain.
