Narrative

While pushing a roof bolt into a hole, a piece of slate (25"" x 27"" x 1.5"") fell from the roof and struck the bolter and broke up when it hit the bolter. A piece of the rock struck the injured just above the left eyebrow causing a laceration that required 5 sutures. No time from work was lost. This is a medical treatment incident.
