Narrative

EE was operating a roof bolter installing supplemental roof supports (12 feet long cable bolts) in an area outby the face areas. As he was using boom of the bolting machine to push the bolt into the hole, bolt caught in the hole bent and rotated, the bolt struck the EE in his right upper arm.
