An unintentional roof fall occurred in the 004 unit intake at 20+00 station in between the #3 and #4 entries. The fall did not affect ventilation, passage of men or cause personal injury. The fall measured 9 feet x 18 feet x 6 feet thick. It had been roof bolted with 6 foot fully grouted roof bolts.
