Narrative

The employee was grading the mine floor with a Joy CM1415 in the 2 Lt. #3 bleeder entry. In the process a piece of roof rock fell unexpectedly between roof bolts and struck the injured on the left shoulder and back. The average height of the No. 3 entry is approximately 9.5'. The employee started missing work on August 4, 2008.
