## Narrative

Shift began 6/28/2008 @ 7:00 p.m.  Employee slipped on grease on haul truck;  caught himself on handrail.  Employee complained of rib pain but released to work.  Employee was taken off work 7/22/2008 due to wrist/shoulder pain.  An investigation to determine if new pain was related to 6/29/08 injury.  Our insurance carrier determined new pain is associated with original injury.
