Narrative

A roof occured in the 3-North Intake Entry. The fall was located in the #12 crosscut in the #4 entry. The fall was 20' by 20' and 8' to 9' in thickness. The area was originally supported with 5' Hytech Bolts. The fall will be cleaned up and re-supported. The fall was immediately reported to MSHA.
