## Narrative

Employee was closing a butterfly valve with an 18"" crescent wrench on the valve stem. When valve broke loose it caused the employee to strike himself under the left jaw. Neck and tooth sore, cut inside lip, possible chip behind front tooth. His neck remained sore; saw doctor on 7/23/08. Follow-up on 7/30, doctor ordered therapy.
