## Narrative

EE was in process of setting turn timbers in the #13 entry of Mains section for the right barrier lift, when two sections of roof broke loose from the roof, measuring from 0-2"" thick and 45"" X 28"" and 45"" X 27"". The roof fell out from around the roof bolts, striking him on back of the head, middle of back, top left hand and left elbow.
