## Narrative

Mechanic was attempting to put in bearing spacers. He hoisted up cylinder with his crane, the cylinder slipped catching his hand & striking his elbow against the tire. The incident was reported to management on 07/21 as a first aid injury. On a followup visit to the orthopedic specialist an x-ray was taken (8/12/08) & a fracture of the rt. forearm was indicated.
