Narrative

Installing a pie shaped piece of 1/4"" plate in the duct work on the CA-2 calciner discharge. The plate hung up and while manipulating the 1/4"" plate, employee fell down the incline plane and fell into the basin, receiving a broken left arm, 3 ribs were broken and a fractured pelvis.
