## Narrative

The employees were pulling miner cable outby with the 913 loader hooked to a wire rope cable that went through a pulley that was chained to a bolt.  The chain broke and the cable struck the employee in the lower back causing a contusion to his back.  He was taken to seek medical attention and was put on restricted duty to finish up his set to allow his back to heal.
