An unintentional roof fall occurred in the old 4-E worked out panel in entry #2 at station 10+25. The fall did not affect ventilation, affect passage of men or cause personal injury. The fall measured 22 feet x 18 feet x 6 to 7 feet high. The area had been bolted with 6 foot fully grouted roof bolts.
