## Narrative

While cleaning an over head duct pipe the bolts holding the mounting bracket broke allowing the bracket to bend.  This caused the pipe to come down striking the right shoulder and knocking the employee to the ground and landed on his left leg causing a laceration requiring surgical cleaning and stitches.
