Narrative

Employee was jackhammering buildup at the base of a bucket elevator. The buildup was 16-24"" tall and was 30"" wide. As he was jackhammering from the top of the buildup to the bottom the material sloughed off towards his feet causing the his hand to contact a 3"" wide flange that caused him to suffer cuts on his finger that required 7 stitiches.
