A roof fall occurred in the #4 entry at the #11 crosscut on the 3-North-1 Mains in the intake entry. The fall was 8' to 9' in thickness and measured 20' by 20'. The area was previously supported with 5' HY-Tech Bolts. The fall was cleaned up and re-supported.
