## Narrative

While bolting the operator had his left hand and arm on top of the canopy holding a 4? guide bolt. As he looked behind him a piece of rock and shale fell from the roof striking his arm leaving a puncture and laceration. Transported by ambulance, X-rays revealed no fractures. The wound was not sutured due to the severity of the swelling.Began missing work 9/23/2008.
