Employee was smoothing cut edges on a 24"" diameter steel plate he had clamped in a vice. The vice was not tightened sufficiently and the plate started moving. Employee let loose of the grinder with his left hand to catch the moving plate - while doing this his wrist contacted the grinding wheel and was cut.
