## Narrative

Employee was replacing the A frame on a pickup truck. He was attempting to remove a bolt which was so tight that an extension was needed. As employee was pulling with both hands on the extension the bolt came free suddenly and the employee felt a pain in his left shoulder/neck area. The location of the bolt prohibits the use of an air wrench or longer extension.
