## Narrative

On or about 9/25/08, EE was climbing down to dismount his loader, tripped on the last step, fell to the ground. He put his left arm out to catch himself. EE now says injury to his left arm. He did not seek medical attention until 10/29/08. His 1st shift from work was 10/30/08.
