Narrative

Employee was opening the back (swing) gate for the crude delivery truck to exit the plant. He was holding the gate and walking to the ""open"" locking mechanism, he did not realize his hand was lined up with the locking mechanism. As he engaged the gate with the locking mechanism, his hand struck the locking mechanism.
