Narrative

While performing routine boiler maintenance on one of the mine's two coal fired steam boilers, the injured welder/mechanic had the unit shut down for cleaning bottom ash/clinker out of the fire box. The mechanic dropped the cleaning tool into the fire box, causing hot ash to blow through the service door onto his face. Resulting in 1st and 2nd degree burns to his face.
