The roof fell in the #12 lift after the lift had been mined and extended out into the entry in front of the #2 MRS. The fall pulled four bolts. A retriever was used to recover the MRS. The roof was previously supported with 5' Hy-Tech bolts. The fall was on 003 section 4-Right Panel between the #11 and #12 crosscuts.
