Narrative

Intersection fell from the face which was extened 10' beyond the last open xcut to 5' outby the last open xcut, Fall was 30' lomg x 17' wide x 8' high. The area was bolted with 5/8x72 fully grouted rebar and 12x12 plates. The following was dangered off and cribbed. It will not be cleaned up, the fall occurred between 5 lifts.
