## Narrative

AT 10:40 am an MSHA Inspector inspecting the mine saw a 2 inch steel braided air hose rubbing on the brow. He declared the Alimak unsafe to operate which is the alternate escape route. The Inspector called his supervisor and at 12:15 pm MSHA decided this was immediately reportable.
