Narrative

Employee was cleaning up the notch area. Out of the corner of his eye he saw a piece of the notch peel & fall. He turned around and apparently ran into the rib or the bore miner but he didn't know which one. The area of the piece that peeled was unbolted and not in his immediate area. A 7000-1 is being filled out due to the accident being immediately reported to MSHA.
