## Narrative

EE was welding a fitting in a tank from a water truck, there was a spotter outside in the tank. The EE took a break and left a propane torch hanging in the tank. When he returned, struck the lighter in the tank and there was an ignition. Post investigation revealed that EE had left the torch valve partially open and had not turned on the fan prior to reentering the tank.
