Narrative

A roof fall occurred in the #5 entry in the 12 south area at x-cut #60. The fall was about 19' by 20ft and 5 to 8ft high. The area was boltes with 5ft torque tension bolts. It was timbered and dangered off, it will not be cleaned up.
