## Narrative

An employee was struck by a rock which dislodged from the mine roof.  Ventilation tubing was needing to be extended and the employee stepped up to the miner to retrieve a drill so a spad hole could be drilled to secure the ventilation tubing.  As the employee stepped back away from the miner, the rock fell from the roof striking him in the head, neck and back.
