Narrative

Employee stated he was out-by the mining section picking up trash and putting it a trash car. After putting a wooden pallet into the car, his shoulder started hurting. It had been bothering hi m some but started hurting worse. He continued to work, then he was taken off work on 12/20/2008. Diagnosis with Supraspinatus tendonosis and Acromioclavicul joint arthritis.
