12/23/2008 employee reported his back felt tight and went to a physician.  Employee could not identify any specific incident that caused his discomfort, he was advised that his claim would not be accepted under workers compensation pending investigation/medical review.  Based on medical report received January 8, 2009 this claim has been accepted as an occupational illness.
