## Narrative

Employee stopped to get samples at the grind sample area , she entered through the walk-through door, it was dark to her as her eyes & glasses hadn't adjusted to the lighting difference. There was a 3 "" x 20' pipe laying in the walkway that she tripped over . As she tried to catch her balance, she landed on her left hand causing a laceration on her left palm.
