## Narrative

A roof fall was found at 7:05 am on 2/9/09 at survey station 12+10 room 2 6rms. Fall was in a four way intersection that had been mined january 2009 and was bolted with six foot full resin pins. a signifigant clay dyke was present in intersection and addition props had been installed in adjacent rooms. No injury or property damage resulted. Fall was reported
