Narrative

Employee involved in incident was walking under a 4' high catwalk that had a conduit running 6"" underneath it. As he was walking under catwalk he bumped the conduit with his head causing a neck strain. He did not seek medical treatment until 2/5/09 and was diagnosed with a neck starin and placed on a scrip. Employee has had three shoulder/neck injuries previously.
