Narrative

Employee stated that she and witness had just finished cleaning 1 north belt at 50 block. The witness attempted to throw a broom across the belt. The broom struck a belt chain and fell onto the top belt. He attempted to reach onto the moving belt for the broom when her left hand and forearm was caught between moving top conveyor belt and a top roller causing laceration 9 stiche
