## Narrative

The employee stated he was bolting and was pushing the bolt wrench onto the bolt and had his hand on the bolt when he accidently hit the rotation on the machine. His glove was caught between the wrench and the roof bolt and this caused the glove to the pulled off his hand and hyperextended his fingers.
