Narrative

While working with other employee relocating 6"" water hoses about 25' long, into a trench in Phase I, employee allegedly twisted and strained his back. Employee worked 8.0 hours the next day, and did not show up for work on 2/9. EMPLOYEE REPORTED THE INJURY TO THE NIGHT SUPERVISOR ON 2/9/09.
