## Narrative

Maintenance work was being performed inside the first chamber of the cement mill #3, several plate liners were being replaced. Employee was welding inside the mill standing on the first rung (bottom) of the access ladder while welding the plates; he stepped down, got tangled on his own boot shoelace & fell backwards, falling over the bed of balls. Employee fell from 12"" high.
