## Narrative

EE stated that while he was installing the inside bolt in #4 entry of 5 Rt future longwall setup face his drill steel plugged up & he dropped the drill head off to check the chuck section of bolter. As he dropped the drill head off the steel stuck in the hole. He placed his rt hand on drill head to check the suction & a section of drill steel fell & struck his rt hand.
