Narrative

Employee had almost completed loading a shuttle car. A rock measuring 91"" x 13"" x 11.5"" fell from top and landed on the boom of the miner. It traveled the conveyor to the shuttle car. Since the car was almost loaded the rock slid off the side of the shuttle car and struck employee in the left leg.
