Employee was initially injured on 6/19/2008 (E-Document #638715). Employee returned back to work on 1/28/2009. Went on his 7 days off on 02/12/2009 and then went to his personal doctor on 02/20/2009. A new 7000-1 is being submitted. We were not notified until 02/24/2009.
