Sometime between the end of night shift (3/31/09) and the start of day shift (4/1/09), the cemented sandfill in the slot and intersection of the 5900-12 Stope failed. The failure was approximately 35 feet long through the intersection and 30 feet into the west stope and 5 feet into the east stope.
