Narrative

EE was profiling the 1 South belt structure using a chain hoist that was hooked into the chain that the belt was hanging from. The EE was standing on the belt which was 60"" off the bottom, when the chain hoist came loose, causing the EE to lose his balance and fall from the belt to the mine floor.
