Narrative

Taking 3rd cut fom #4 entry, squatted down, roof soap stone, rock fell from between rib and bolt striking employee's right shoulder area. Taken to ER for treatment, x-ray negative. Saw orthopedic 3-31-09, diagnosed with contusion to right rotator cuff and right collar bone.
