Narrative

On 3/31/09 the employee was kneeling down using a water hose to clean material from under a belt pan. The hose blew out, causing the employee to be startled and strained his left hamstring in the process of getting out of the way. The employee started missing work on 4/2/09 for a severe left hamstring strain.
