## Narrative

Two employees were installing 8"" pipe. While tacking pipe in place, a spark entered a piece of PVC stub protruding from floor. This spark ignited the PVC glue gasses that were trapped in the piping system, creating an explosion that sent PVC pipe fragments throughout the plant. Two employees received medical treatment for shrapnel injuries.
