Narrative

Employee was installing roof bolt and guiding it into the roof bolter drill chuck when he bumped the rotation lever to seat the bolt his glove caught onto the bolt wrapping his hand around the bolt causing a ""mallet finger"". Employee continued working regular job until 04/28/09 when he had minor surgery on the finger.
