Narrative

EE was attempting to cross the 5B belt at the 5B flow-thru tail piece and mistakenly turned belt switch to the ""on"" position and was carried into the tailpiece. His right foot then became lodged between the skirt board and impact roller, causing a sever laceration to his foot.
