Unintentional roof fall: d-return, No. 1 entry, crosscut no. 32, survey station no. 10183. The fall measured approx. 40 ft long , 20 ft wide and 5 ft thick. Five foot 3/4"" ida. fully grouted resin roof bolts were in use as primary roof support.
